How Do I Enroll?


Enrolling

It is easy. If you qualify, you can enroll at any time in the iCare Medicare Plan.

You could be eligible if you:

  • Live in our service area of Adams, Brown, Calumet, Columbia, Crawford*, Dane, Door*, Fond du Lac*, Green, Green Lake, Iowa, Jackson, Juneau, Kenosha, Kewaunee*, La Crosse*, Lafayette, Manitowoc, Marinette, Menominee, Milwaukee, Monroe, Oconto, Outagamie, Ozaukee, Racine, Richland, Rock, Sauk, Shawano, Sheboygan, Trempealeau, Vernon, Walworth, Washington, Waukesha, Waupaca, Waushara*, and Winnebago counties.
    *These counties effective 1/1/2018.
  • Are eligible for Medicare and Medicaid benefits OR eligible for Medicare cost-sharing assistance under Medicaid
  • Have Medicare Parts A and B
  • Do not have End-Stage Renal Disease. (Exceptions may apply)

Call us at 1-888-241-9659 to see if you qualify for the iCare Medicare Plan HMO SNP. Calling this number will connect you with a Medicare Benefits Consultant who can help you find out if you qualify for the healthcare benefits and services provided by the Independent Care Health Plan.

Enroll through an in-home visit or over the phone by calling

Or you can simply complete our Enrollment Form and mail it to:

After your eligibility has been determined, we will send you a membership packet explaining how to access your iCare Medicare Plan Benefits And Services.

For more information contact our Customer Service department at 1-800-777-4376 (TTY 1-800-947-3529), 24 hours-a-day, 7 days-a-week (office hours: Monday-Friday, 8:30 a.m. to 5:00 p.m.). You must enroll in both the iCare Medicare Plan as well as a full Medicaid HMO plan to receive full benefit coverage. You can also remain in Medicaid Fee-For-Service, but you will be subject to small co-pays if you do so. To find out about a full Medicaid HMO Plan that is best for your health care needs you may contact the Medicaid Managed Care Enrollment Specialist at 1-800-291-2002 (TTY: 1-800-325-0778).

Independent Care Health Plan (iCare) is a Medicare Advantage HMO SNP organization with a Medicare contract and a contract with the State Medicaid program. Enrollment in any iCare plan depends on contract renewal. Plans are available to anyone who has both Medical Assistance from the State and Medicare. This information is not a complete description of benefits. Contact the plan for more information. Limitations, copayments, and restrictions may apply. Benefits, premiums and/or co-payments/co-insurance may change on January 1 of each year. You must continue to pay your Medicare Part B premium. The Part B premium is covered by the State if you are a full-dual member. Premiums, co-pays, co-insurance, and deductibles may vary based on the level of Extra Help you receive. Please contact the plan for further details. The Formulary, pharmacy network, and/or provider network may change at any time. You will receive notice when necessary.
